Please only request a blood test appointment if it has been requested by the surgery. Arranging an appointment can be done via 2 methods.
Option 1: In House Phlebotomy Service
Please call 01926 316711 to arrange your blood test appointment if you have received correspondence from us requesting you to complete a blood test
Due to high demand as a result of COVID19 it may be difficult to fit you in promptly as there is a waiting list. Therefore if you prefer you can try option 2.
Option 2: Warwick & Stratford Hospital Phlebotomy Service
Book your appointment online
Once you have completed your bloods please return and complete a medication review if requested to via text. If there are any abnormalities on your tests we will get in touch via either text, letter or phone call. You can view your results via online access.
